Liposuction, the ‘Yo-Yo Effect’ and Intermittent Fasting Explained!

Otmaro Belalcázar, an expert in internal medicine and endocrinology, is one of the most recognized specialists in Colombia. His rapid rise to prominence is partly due to his extensive patient list, which includes several well-known figures in the country who have praised his treatments.

In a recent conversation with Sin Filtro in SEMANA, Dr. Belalcázar discussed essential information for maintaining healthy habits and knowing when to consult a physician. One topic that generates significant confusion in Colombia is liposuction, often mistakenly sought as a weight-loss method.

Dr. Belalcázar clarified the nature of the liposuction procedure and explained why it may not always yield successful results, emphasizing that the responsibility does not lie with plastic surgeons. “Liposuction is a beautiful surgical and aesthetic procedure that improves aesthetics by removing internal fat. However, the issue with metabolic diseases is not the external fat, but the internal fat,” he stated.

He further explained, “When plastic surgeons remove external fat, they are essentially clearing out a storage of fat. If lifestyle habits are not changed and proper care is not taken, that storage will refill. This leads people to believe the procedure was ineffective.”

Dr. Belalcázar noted that many individuals who undergo liposuction continue to maintain poor dietary habits. “If people do not exercise and indulge in unhealthy foods, such as a bandeja paisa, the results will not be effective. While body aesthetics can be improved, there must be a commitment to care.”

He also addressed the concept of “weight rebound” that can occur after diets or procedures. “The importance of preventing weight regain relies solely on the individual. It is crucial to change habits to avoid regaining weight. Aim for about 7,000 steps daily, engage in strength training, and maintain good nutritional habits.”

The specialist emphasized that “medication is not the solution, nor is bariatric surgery. These are aids, but habit changes are essential; rebound weight gain occurs when good habits are not adopted.”

Regarding fasting, Dr. Belalcázar stated that it can be beneficial, but he does not advocate skipping breakfast due to potential complications. “Scientific evidence shows that fasting can yield similar weight loss results. The best time for fasting is during the night.”

He explained that “a fast should ideally last about 12 hours, with 14 hours being excellent and 16 hours exceptional. Skipping breakfast can lead to hormone release that raises glucose levels, which can be detrimental. Fasting is acceptable, but breakfast should not be skipped.”

Dr. Belalcázar shared his personal approach: “I breakfast like a king, lunch like a prince, and dine like a beggar. That is the secret. Fasting can help eliminate unwanted sensations.”

He added that breaking a fast with carbohydrates is illogical. “It makes no sense to end a fast with carbohydrates. For instance, breaking a fast with a bag of chips is detrimental due to its fat, preservatives, and salt content.”
